Step 1: Setup your software
Download your Transform app:
Reset your password: After you sign in for the first time, you can change your password by tapping "Reset my password". You will receive an email from Transform Support within a few minutes with instructions to change your password.
Start tracking your food using the food and beverage tracker built into the app. Your coach will be able to view your food entries and provide tailored feedback.
Start tracking your activity using the activity tracker. All of your weekly active minutes will be displayed in a graph on your dashboard to help you better reach your goals.
Start chatting with your fellow Transformers through the Transform Group chat.
1. First, download the Fitbit App
Set up your Fitbit account.
2. Then, connect your Fitbit account to your Transform account
Open your Transform app
Tap “Connect my Fitbit” (scroll down a bit)
Enter your Fitbit login information. IMPORTANT: This step requires the login credentials that are linked to your Fitbit account and NOT your Transform account.
3. Finally, start tracking your activity
Contact our Support Team for any issues with your Transform app.
Step #2: Set up your hardware
1. First, download the Nokia Health Mate App
Set up your Nokia account.
2. Then, connect your Nokia account to your Transform account
You’ll need to connect your new Nokia account to your Transform account so that your weight measurements automatically appear in your Transform app and on your coach’s dashboard.
Open your Transform app
Tap “Connect to Nokia” (scroll down a bit)
Enter your Nokia login information. IMPORTANT: This step requires the login credentials that are linked to your Nokia Health Mate account and NOT your Transform account.
3. Connect your Nokia Body scale to your mobile device

Remember that this scale is connected to your Transform account, so if someone else steps on it, their weight will show up on your app! Try to keep the scale just for yourself.
The Fitbit is an activity tracker that you wear on your wrist. It tracks your steps, active minutes and calories burned. Your health coach will also be able to see your Fitbit data and can give you feedback and guidance on increasing your physical activity.
You’ll need to earn your Fitbit by:
Stepping on the scale at least three times each week.
Completing your one-on-one coaching session each week.
Posting to the Transform Community Group
Completing the weekly lessons and quizzes.
Tracking your food in your Transform or Fitbit app.
Once you’ve earned your Fitbit, you will receive an email from Solera Health with instructions on how to claim your Fitbit.
